Fentanyl Eradication Project
Phoenix Wellness Center is a low-barrier culturally specific substance use outpatient facility. It was founded because of the shared passion founders have to help those in our community struggling with addiction. In 2024 our community suffered many losses due to overdose and from that pain, sorrow and determination to make it stop; the “The Call to Action Team” was born. The Call To Action Team is a collaborative group made up of community organizations that share the common goal of eradicating fentanyl and ending overdose in our community.
