In order to sign up for services we need the following information to get started:
Full Legal Name, Date of Birth, phone number, email address.

Once that information has been input in the computer the receptionist will send you an email with documents to complete. Feel free to call the front desk for step by step directions or email us at info@phoenixwellnesscenter.org

Our COVID-19 Response

We want you to know that here at Phoenix Wellness Center we are taking all of the necessary precautions as recommended by both the Center for Disease Control (CDC), and Substance Abuse & Mental Health Services Administration (SAMHSA).

Due to the highly contagious COVID-19, and our recent increase in Lincoln County positive cases; we are utilizing both telehealth and in-person services.

Groups therapies are done via google-meet and individual sessions can be done using the most applicable form of communication, available to you – consult with your counselor.
